When receiving medical treatment, present your MISHIP Identification Card AND the appropriate picture identification (passport, driver's license, or student identification card) to the physician when receiving treatment.
Take a Health Claims Form with you to your appointment. The physician's office or hospital may decide to send the claim directly to Great-West Life.
If the physician's office or hospital requires you to pay for medical services out-of-pocket, please obtain official receipt of payment from the provider and submit a claim to Great-West Life for reimbursement.
