Receiving Medical Attention

CLAIMS
STUDENTS

When receiving medical treatment, present your MISHIP Identification Card AND the appropriate picture identification (a valid passport, driver's licence, or student identification card) to the physician when receiving treatment.

Take a Great-West Life Health Insurance Claims Form with you to your appointment. The physician's office or hospital may decide to send the claim directly to Great-West Life.

If the physician's office or hospital requires you to pay for medical services out-of-pocket, please obtain the official itemized receipt of payment from the provider (which includes tariff code details). In the absence of a detailed receipt, ensure that the physician's office completes Part 3 of the Great-West Life Health Insurance Claim Form (continues on the backside of the form).

You can then submit the claim to Great-West Life for reimbursement. See Making a Claim for further information.

IMPORTANT!
Submit your claims as soon as possible. Great-West Life may not be liable for expenses if they receive your claim more than 90 days after the date services or supplies are provided.
