Making a Health Claims | Making a Prescription Drug Claim

 

MAKING A HEALTH CLAIM

    1. Print a Great-West Life Health Insurance Claim Form (pdf)
    2. Collect your original receipts
    3. Complete the form. Include your:
        1. current home address
        2. school information
        3. if applicable, dependent information
    4. Please ensure that your physician or service provider completes all appropriate sections of the claim form. (You should take the claim form with you to your medical appointment.)
    5. Include your personal ID Number from your MISHIP Identification Card
    6. Sign the claim form (keep a copy of the form and your receipts for your records)
    7. Mail the signed form and your original receipts to:

    1. Print a Great-West Life Prescription Drug Claim Form (pdf)
    2. Collect your original receipts
    3. Complete the form. Include your:
      1. current home address
      2. school information
      3. if applicable, dependent information
    4. Include your personal ID Number from your MISHIP Identification Card
    5. Sign the claim form (keep a copy of the claim form and your receipts for your records)
