Making a Health Claims | Making a Prescription Drug Claim

| Submit your claims as soon as possible. Great-West Life may not be liable for expenses if they receive your claim more than 90 days after the date services or supplies are provided. |
MAKING A HEALTH CLAIM
- Print a Great-West Life Health Insurance Claim Form (PDF) and bring this form with you to your medical appointment. Please ensure that your physician or service provider completes all appropriate sections of the claim form (in the absence of a detailed receipt).
- Collect your original receipts from your service provider.
- Complete the remaining sections of the claim form. Include your:
- personal ID Number from your MISHIP Identification Card
- current home address
- school information
- if applicable, dependent information
- Sign the claim form (keep a copy of the form and receipts for your records)
- Mail the signed form and original receipts to:

MAKING A PRESCRIPTION DRUGS CLAIM
- Print a Great-West Life Prescription Drug Claim Form (PDF).
- Collect your original receipts from your service provider.
- Complete the claim form. Include your:
- personal ID Number from your MISHIP Identification Card
- current home address
- school information
- if applicable, dependent information
- Sign the claim form
(keep a copy of the claim form and your receipts for your records)
- Mail the signed form and original receipts to:
